Anal Fissure

About Anal Fissure

A fissure is a tear of the skin at the edge of the anus. It can cause pain, bleeding and/or itching.

One may have ‘burning or pain during defecation’, which last for 1-2 hours after defecation. Along with this, bleeding may or may not present. An extra growth of skin in the mid line in front or back of the anus hangs out, which is felt while washing the area. Women after delivery commonly suffer from this.

Problems

Main cause of this disease is passage of hard stool, which is tearing the delicate anal canal. This forms a wound or cut in front or back part of the anus, results in severe pain. Once damage occurs here, due to hard stool, it fails to heal. This is because daily it contaminates with stool. Further injury and bacterial contamination, un-cleanliness makes wound further worsen and fails to heal. Then it is called chronic fissure. Patient will have pain or burning sensation for months together. Though it heals with great difficulty, again passage of hard stool may again cut or injures the area and again there its formation of ulcer. This is why patient with fissure have repeated attack of symptoms. These types of attacks may last for years in some women.

Symptoms

  • An anal fissure causes a sharp, stinging, or burning pain during a bowel movement. The pain, which can be severe, may last for a few hours.
  • Fissures may itch.
  • They often bleed lightly or occasionally cause a yellowish discharge

Diagnosis For Anal Fissure

Most health professionals can diagnose an anal fissure by listening to the symptoms and by looking at the anus. Usually, the health professional can see the fissure by gently separating the buttocks.

A health professional may use a gloved finger (digital rectal examination) or a lighted instrument (anoscope) to examine the fissure. However, if the fissure is extremely painful, the health professional will usually wait until it has begun to heal before performing a rectal exam or using an anoscope (anoscopy) to rule out other problems. A topical anaesthetic may be used if an immediate examination is necessary.

Causes

  • An anal fissure is usually caused by a hard and dry bowel movement, alongwith a tight anal musculature, which tears the anal lining.
  • Trauma also can cause fissures. During childbirth, 11% of women develop anal fissures. Fissures can also be caused by digital insertion, foreign body insertion, or anal intercourse.
  • Anal fissures affect people of all ages, particularly young and otherwise healthy people. They are equally common in men and women.
  • A low-fibre diet may play a role in the development of fissures.
  • Other causes include diarrhoea and inflammation of the anorectal area.

Treatment

Taking only oral medications may helps in some patients if fissure is of recent orogin. Final and permanent remedy for this is again kshara karma. Under Suitable Anesthesia application of kshara to fissure bed. This burns the part mildly without damaging much tissue. So that fresh wound is created which heals faster. Agni karma follows this, senile pile excised and sphincterotomy is adopted to prevent recurrence. Intermittent anal dilatation to be done after the procedure in further follows up.

What are the precautions?

  • Avoid constipation.
  • Drink lots of water.
  • Do not consume dry and hard food.
  • Have fibre rich diet. E.g. leafy vegetables, raw vegetables and fruits etc.
  • Consume 8-10 water soaked dry grapes before going to bed.
  • Drink a glass of hot water soon after you wake up in the morning.
  • Do not sit for longer time while passing motion & do not strain.
  • Sit in hot water for 10 mins if suffering from thrombosed haemorrhoids. Consult a surgeon soon.

Rules & Diet ( pathya) in Ano-rectal diseases:-

  • Following pathya during treatment is as important as the treatment.
  • Diet is for curing disease not for the medicine prescribed. One does not require treatment if diet (pathya) is properly followed. Medicines do not act if pathya is not followed properly.

Food which are to be avoided:-

  • Dry and hard items which are difficult for digestion.
  • Consuming food more than the requirement.
  • Excess meat, spice, fried items, chilly.
  • Alcohol & smoking.
  • Improperly cooked vegetables.

Prohibited actions:-

  • Sitting for long duration during passing motions.
  • Straining during passing motion.
  • Forcefully initiating motion.
  • Excess travelling, specially on two wheeler.
  • Sleep in the day time.
  • Sitting for the longer (during) duration in same place.

Recommended diet and rules:-

  • Use churned butter milk.
  • Drinking 1-1.5 lts of lukewarm water every morning.
  • Avoid indigestion & constipation.
  • Use- old rice, wheat, ragi, honey, amla candy, dry grapes, pomegranate, ripe banana, musambi.
  • Consume variety of vegetables after proper boiling.
  • Consumption of fibrous food e.g. carrot, cauliflower, radish, bitter gourd, ladies finger, spinach

Regular mild exercise:-

  • pavana muktasana.
  • Ardhapavana muktasana.
  • Pranayama & etc.
